
NHS, Local Authority and third sector staff are working harder than ever to get services back on track, to get waiting lists down and consistently deliver the best care.
We know change is needed. But we also know that many of the solutions we need are already here, working somewhere in the NHS today.
Bath and North East Somerset, Swindon and Wiltshire Integrated Care Board (BSW ICB) is already doing great work in this area and is supporting the NHS to tackle these challenges head-on. BSW ICB is doing this through initiatives such as the Care Coordination project to coordinate health and care services so that patients avoid emergency admission, increasing the number of patients able to receive a high level of acute care at home through our Hospital at Home Virtual Wards scheme and helping members of farming communities access health and wellbeing support through outreach work.
BSW ICB and the NHS know there is much more to do, which is why this consultation for the 10-Year Health Plan is so important.
